There must be dozens of options. One that just came to mind is
OmniOutliner. With it you could create an outline and add columns to
it for all of your important criteria. Some of the columns could be
made a drop down list that you customize, another column for Date
which allows date formatting, etc. It really very flexible and
beautiful to look at too, which I think is important since you'll
probably be looking at it a lot.
You could even then organize the outline by client, easily view by
client etc.

It would seem you could make a simple database using FileMaker Pro or
NeoOffice, to do what you need, if there is nothing out there that's
cheap/free.

Most TidBit readers would use straight text. I'd use Mind Mapping
software because it is that much easier to add comments in the middle
of blocks of text; rearrange; see the wood from the trees and to
see relationships with other items on the map, or to imagine them
when that data is elsewhere.
It is not necessary to write out all the text to all messages to make
sense of the MMp. It can be quick to draw and to read: unless you
need the full transcript for business integrity... Then there are
branch notes that parallel outliners; many MMp software packages
convert MMps to an outline in one click, and / or as an export...

Both Mac / Win.
If you do prefer pure text to graphic display I shall not try to
persuade you to change; but 40% of the world can grasp the graphic
display while a much smaller number really 'Get' the text.

In line with the previous suggestions, I also don't think you need a specialized application. Something with a good self-made template would work. Two that I would particularly recommend are Circus Ponies Notebook, which has a useful indexing feature, and VoodooPad, which works like a wiki and has useful tagging and search features. Both are fairly inexpensive and well documented, and both come from courteous and helpful developers.

The Missing Sync for iPhone might provide some useful features with
its Call Log feature.

Or the neat little Ruby script iphonelogd.rb ^W^W^W calllog2ical.rb:

It parses the iPhone backup iTunes creates every time you sync, and
turns the call log into entries in iCal, including who the call was
with (by name, if they were in the phone's address book), whether it
was a "call from" or "call to", the duration of the call, and the
telephone number in the URL field.
All you'd have to do from there would be to plug in any notes
pertaining to the call.
The only catch is that it (currently) has to be run by hand from
Terminal, but the documentation implies that they're going to hook it
to launchd, so that it will either run periodically, or better still
can possibly be triggered automatically whenever you initiate a phone
sync.
